The SchoolBridge system allows you to cater to the needs of your school for both internal requests (staff leave of absence requests, resource requests) as well as external requests (Education Outside of the Classroom - EOTC). We are now partnered with EONZ to provide government approved forms.

Basic Steps: Check your Request Settings => Create/edit a Request Category Form => Create/edit a Permission Slip Form => Create/edit a Risk Identification, Risk Assessment Form

Request Settings

Check your Request Settings Log in to SchoolBridge: https://yourschoolswebsite/administration

Navigate to "SchoolBridge Settings" on the left hand side navigation bar.
Select "Request Settings" in the sub menu.
Here you can set an email for test mode, modify your thanks content for your Permission Slip and allocate which Caregivers you would like to send alerts to.
Don't forget to uncheck the Test Mode check box when you are ready to send your modified email.

Request Forms

Request Types There are, by default, three Request Types to select from: Education Outside of the Classroom - EOTC Teacher Professional Learning Request - PLD Application for Leave - Leave Requests

How to create a new Request Category Form These forms will be selected by the Staff when submitting their request forms

Log in to your admin site: https://yourschoolswebsite/administration
Navigate to "Requests and Forms" on the left hand side navigation bar
You should now see your "Manage Request Form Categories" page.
Click on the "New Category" button on the top right
Select your Category Type and complete the fields.
Click "Save".
Click on "Build Request Form" and add fields for the Staff to complete when submitting a request. Save changes.
Complete the form by setting up your chain of approval. After a staff member submits their above request the request will be sent to each person within each stage. Each stage can have one or multiple people assigned but only requires one approval or decline action for the request to progress to the next stage.
Save Stages.
To view your new category, navigate to "Requests and Forms" in the left hand side bar, this will display your "Manage Request Form Categories" page

How to edit a Request Category Form 1. Log in to your admin site: https://yourschoolswebsite/administration 1. Navigate to "Requests and Forms" on the left hand side navigation bar 1. You should now see your "Manage Request Form Categories" page. 1. Click on the "Edit" button. 1. Save Changes. 

Permission Slips

How to create a new Permission Slip These forms will be selected by the Staff when submitting their permission slip

Log in to your admin site: https://yourschoolswebsite/administration
Navigate to "Requests and Forms" on the left hand side navigation bar
Select "Permission Slips" in the sub menu.
Click on the "Create New Form" button
Complete the fields.
Click "Save".
Scroll down to "Form" and add fields for the Staff to complete when submitting a request.
Preview your form by clicking the "Preview" button on the top right
Reorder your fields by clicking the "Drag Mode" on the top right
Save Changes.
To view your new category, navigate to "Requests and Forms" in the left hand side bar, this will display the "Manage Request Form Categories" page.

How to preview/edit/duplicate a Permission Slip Form

Log in to your admin site: https://yourschoolswebsite/administration
Navigate to "Requests and Forms" on the left hand side navigation bar
Select "Permission Slips" in the sub menu.
To preview your form, click on the "Preview" button on the top right
To duplicate your form, click on the "Duplicate" button on the top right
To Edit your form, click on the "Edit" button on the top right
Click on the "Edit" button.
Save Changes.

Risk Assessment Forms

**How to create a new Risk Assessment Form**

These forms will be selected by the Staff when submitting their permission slip

Log in to your admin site: https://yourschoolswebsite/administration
Navigate to "Requests and Forms" on the left hand side navigation bar
Select "Manage Risk Assessment Forms" in the sub menu.
Click on the "Create New Form" button
Complete the fields.
Click "Save".
Scroll down to "Form" and add fields for the Staff to complete when submitting a request.
Preview your form by clicking the "Preview" button on the top right
Reorder your fields by clicking the "Drag Mode" on the top right
Save Changes.
