Using the Form Builder

The SchoolBridge Form Builder is an easy to use tool that allows you to create internal request forms, RAMS forms and caregiver destined permission slips.

The process of creating a form involves deciding what information you need to collect and adding the appropriate form fields to allow the user of the form to enter that information.

Category Type
Choose the Type of from this is. If you need to create additional form types, in the main menu click SchoolBridge Settings > Request Form Types to add additional types.

Form Name
Give your form a short descriptive name.

Request Name
Every form created includes a mandatory field called 'Request Name'. This allows the user to give the request they are submitting a title. This field will appear at the start of every form.



Customise Request Name Label
You have the option of changing the label for this field from 'Request Name' to anything you wish. For example, if you were creating a request form specific to sports, you could use 'Sport Name'.

Description
Give your form a short description as to its use.

Form Instructions
Use this field to add detailed instructions for filling out this form or add info about policies relating to it. Effective use of this feature can help make sure your users enter the information that you need. You can include as much detailed as you want here, and include links to external information as well.

Status
Setting the status to of the form to 'Active', allows it to be used by your staff. Setting the status to 'Inactive' keeps the form in the system, but presents users from seeing it.

Activity Log
Tick this option if you would like to keep a record Show request in PLD activity log

Information Fields
These fields are used to display information to the user.

Large Heading - adds a large heading to your form.
Small Heading - adds a subheading to your form.
Description Text - add a paragraph of text to your form.
Inset Description Text - add an inset paragraph of text to your form.

Image
This field-type allows you add an image to your form. You could use this to add your school's logo to the top of your form by adding this field-type as the first item on your form.



User Input Fields
These field-types are used to gather information from your users. You have the option to make any of these fields required, by ticking 'Make Required Field'.

Short Text Field
This field-type allows user to enter a single line of text.

This field-type allows user to enter a paragraph.

Drop Down List
This field-type allows the user to choose one item from a list.

Select One Item
This field-type allows the user to select one item from a set of options.

Select Multiple Items
This field-type allows the user to select multiple items from a set of options.

Check box with Extra Information
This field-type allows the user to add additional information.

Date Picker
This field-type allows the user choose a date from a calendar.

Attach Existing Risk Assessment Forms
This field-type allows the user to attach Risk Identification, Assessment and Management forms to this request

Select Existing Permission forms
This field-type allows the user to add existing caregiver permission slips to this form.

This field-type allows the user to choose multiple staff members from KAMAR.

Choose a single HOD staff Member
This field-type allows the user to choose a single HOD staff member

This field-type allows the user to choose multiple HOD staffs members.

This field-type allows the user to choose a single person from the senior leadership team.
